Brief description of study
The purpose of this study is to evaluate a comprehensive model for integrating both primary and specialty Palliative Care into nursing facilities.
Detailed description of study
Facility staff and family members of patients will be asked to complete a series of questionnaires at baseline and every 6 months for an 18-month period. Family members will receive a $25 gift card after completing the final questionnaire.
Eligibility of study
You may be eligible for this study if you meet the following criteria:
- Conditions: Alzheimer's Disease

Age: Between 18 Years - 100 Years

Gender: All
Facility staff and family members of patients who are long-stay residents in an enrolled nursing home, with moderate to severe ADRD.
