Brief description of study
The purpose of this study is to build a RedCap database to keep a retrospective and prospective log and data of the patients treated with Focused Ultrasound (FUS) Thalamotomy and Pallidotomy which will be provided clinically. The data in the RedCap database will be used for the analysis of the quality, efficacy, and safety of FUS Thalamotomy and Pallidotomy in patients with medically refractory essential tremor and medically refractory Parkinson’s disease.

The sole purpose of this study is to build a RedCap database to keep a prospective log and data of the patients treated with Focused Ultrasound (FUS) Thalamotomy and Pallidotomy which will be provided clinically.
