Brief description of study
Have you or a loved one been hospitalized within the past year? We would love to get your perspective on your discharge experience!
Researchers and pharmacists at the Rx-SafeNet would like to invite you to participate in one of several virtual town halls across northern, central, and southern Indiana. Town Halls will include patients, caregivers, and pharmacists from different sectors in a safe and inclusive environment, encouraging discussion about diverse perspectives on transitions of care (your discharge experience).
Rx-SafeNet's mission is to improve medication safety and advance community pharmacy practice in Indiana through the conduct and dissemination of collaborative, patient-centered, practice-based research.
Detailed description of study
What will happen during the study?
- You will be asked to attend at least one online (virtual) town hall session (90-minutes each)
- You will need to complete a survey before each virtual town hall.
Incentive/compensation
- To show our appreciation for sharing your experience, you will receive $50 after attending one of our town hall sessions.
Eligibility of study
You may be eligible for this study if you meet the following criteria:

Patients or Caregivers of those who have been discharged within the last 6 months who are willing to share their experience.
