Brief description of study
The purpose of this study is to learn more about thoughts and experiences completing the FAST, a tool healthcare providers us as part of a fertility consult. This information may help us better incorporate the FAST into fertility consults at other healthcare institutions.
Detailed description of study
Patients and caregivers who have completed, or at least partially completed, the FAST, which is delivered as a part of a standard fertility consult and is intended for adolescent males (13-21) newly diagnosed with cancer and their caregivers.The study team at NCH will monitor enrollment of eligible participants and will communicate with the site PIs/study teams to facilitate recruitment at their respective sites.
Clinicians at all sites will offer the FAST as part of clinical care during/after the fertility consult and before treatment initiation. Embedded at the end of the FAST will be a message that a research study is examining perspectives and experiences using the FAST as implemented, with the option to opt out of future contact regarding this research study. Participants will complete the single survey that contains questions about their background, sperm banking decision, decision satisfaction, and experiences using the FAST. The survey will take about 5-10 minutes to complete.

Inclusion Criteria:
Adolescent and caregiver participants must meet all the inclusion criteria to be eligible for the study. The inclusion criteria are:
• Completed, or at least partially completed the FAST, which is intended for the following population and their caregivers:
• Male
• Aged 13-21 years
• New cancer diagnosis
• Pubertal (at least Tanner stage 2-3, eligible for sperm banking as determined in the Oncofertility consult)
• English-speaking
This study investigates the use of a tool called the Family-Centered Adolescent Sperm Banking Decision Tool (FAST) during fertility consultations. The purpose is to understand the thoughts and experiences of adolescents and their caregivers who have completed or partially completed the FAST. This tool is part of the fertility consult process for adolescent males who have been newly diagnosed with cancer.
Participants in this study will be asked to complete a survey about their experiences with the FAST. The survey includes questions about their background, their decision regarding sperm banking, their satisfaction with that decision, and their overall experiences using the FAST. It will take about 5-10 minutes to finish the survey.
- Who can participate: Participants must be male, aged 13-21, newly diagnosed with cancer, and at least Tanner stage 2-3. They must have completed or partially completed the FAST and be English-speaking.
