Brief description of study

Hospital at home is being implemented as an alternative post-operative care delivery system for selected colorectal patients who meet medical and geographic criteria to be discharged directly home to the hospital at home program after surgery. The purpose of this study is to gain insight into the patient experience of hospital at home by conducting a mixed-methods study with surveys and semi-structured interviews to collect patient-reported outcomes of the experience. Also, to understand the provider experience we will conduct focus groups of providers involved in the program.

THIS STUDY IS ENROLLING BY INVITATION ONLY - Patients are identified for the HaH program initially by the colorectal surgery team, who determine they are potential appropriate candidates undergoing the approved procedures. At this time their information is given to the HaH team as a referral for enrollment in the program and they determine if the patient is eligible to proceed based on co-morbidities, medical needs, and geographic location. If the patient is successfully enrolled in and participates in the HaH program after surgery, they are eligible for participation in the study. Participants in the focus group portion of the study will be determined to be eligible if they are members of the colorectal surgery team (surgeons, APPs, clinic nurses) or take care of colorectal surgery patients in the HaH program (HaH nurses, team members).

Detailed description of study

The study will focus on two primary groups: patients and healthcare providers. 

  1. For patient-level data collection, there will be two points of interaction. After patients consent to participate and are enrolled in the Hospital-at-Home (HaH) program, they will complete a survey near the time of discharge to assess their initial experience. Approximately 3–4 weeks after discharge, participants will be contacted for a semi-structured interview conducted via phone or video call to gather qualitative insights into their recovery and overall experience with HaH care.
  2. For provider-level data, once the pilot study is complete and data saturation has been achieved, a focus group will be conducted. This session will include surgeons and other members of the surgical care team involved in the HaH program and will explore their perspectives and experiences delivering care within this model.

  • Conditions: Colectomy, Ileostomy reversal, Colostomy reversal
  • Age: 18 years - 100 years
  • Gender: All

Inclusion criteria:

  • Patients who underwent colorectal surgeries eligible for HaH including colectomy, ileostomy reversal or colostomy reversal that participate in the HaH program.

This study investigates the experiences of patients and healthcare providers with a hospital-at-home (HaH) program for individuals undergoing elective gastrointestinal surgery. The purpose of this study is to understand patient experiences by collecting data through surveys and interviews. The study also aims to gather insights from healthcare providers through focus groups.

Patients who participate in the HaH program will complete a survey near the time of discharge and will be interviewed 3–4 weeks later to discuss their recovery and experience. Providers involved in the program will participate in focus groups to share their perspectives on delivering care within the HaH model.

  • Who can participate: Patients who have undergone colorectal surgeries like colectomy, ileostomy reversal, or colostomy reversal in the HaH program are eligible. Healthcare providers involved in the HaH program for these patients are also eligible.
  • Study details: Participants will complete a survey after discharge and join a follow-up interview 3–4 weeks later. Providers will participate in focus groups to discuss their experiences with the HaH program.
